There are four separate tests, so you need to designate if you wish to do 1, 2, 3 or all 4 tests. You can purchase your test by calling our Customer Service Desk at (508) 826-3551. You can also just pick up your kit at your next appointment at one of our clinics.
The instructions are enclosed in your kit for collecting your cheek swab sample and how to mail it in the pre-paid package. Once you have collected your sample, please follow the enclosed directions to fill out your information online at our secure lab website. Then simply drop your package in the mail and your test results with our personalized email review will be sent to you in two weeks.
